Kindergarten Entrance & Age Requirements

Enrollment Requirements

Children eligible for kindergarten enrollment: 

  • must be five years old on or before September 1. (Florida Statutes 232.01 and 232.04) 

  • a birth certificate or other legal evidence of date of birth must be shown and recorded by the school before registration is 
    completed. 

  • Children who attain the age of six years by February 1 of the current school year must be enrolled in school according to the 
    compulsory provision for school attendance 

  • immunization record on the Florida blue-colored HD680 form*. 

  • a record of a recent health examination conducted within the past year recorded on a Florida physical form* 

  • proof of residence address 

  • and all students registering to attend public school for the first time in the county are asked to bring their social security 
    card with them. 

*Physical examination forms (HD Form 3040 March, 91) for a student’s health examination and (HD Form 680) for certification of immunization are available in local doctors offices or from the Volusia County Health Department.

Immunizations

  • Five DPT (Diptheria-Pertussis-Tetanus) immunizations are required unless the fourth immunization is given on or after the fourth birthday. 

  • Four polio immunizations are required unless the third polio immunization is given on or after the fourth birthday. 

  • Measles, Mumps, and Rubella: The combined MMR vaccine is preferred to assure immunity to all three viruses: measles, 
    mumps, and rubella. The first MMR is given at 12 months of age or older. Proof of two doses of measles vaccine are required 
    and enforced for all children entering kindergarten through sixth grade (public and non-public schools) in the fall of the school 
    year. 

  • Hepatitis B Vaccine: completion of the hepatitis B vaccine series (three doses) is required for kindergarten-first grade entry or attendance. 

  • Varicella Vaccine: (Chicken Pox) effective with the 2001-2002 school year
